Eligibility requirements for this service are:

For clients aged 65 years or over please contact My Aged Care
For clients aged 25 – 65 please complete an online referral or contact intake/triage

When to access the service

Service hours are Monday through Friday, 9am – 5pm.

As we are a home visiting service, our clients must reside within the following areas:

City of Bayside
City of Glen Eira
City of Port Phillip
City of Stonnington
