About Integrated Team Care

Through better access to coordinated and multidisciplinary care, the Integrated Team Care Program improves health outcomes for Aboriginal and Torres Strait Islander people. The ITC program team – made up of a Care Coordinator and Aboriginal Outreach Workers – collaborate with GPs, specialists, community services, and Aboriginal and Torres Strait Islander patients. Working together, they can increase health education, improve access and connections to health and community services, improve health outcomes and manage chronic disease.

GPs can refer patients to the program for access.
